Aged Care Provider

Hudson is recruiting on behalf of an independent reputable not for profit organisation that provides high quality services for elderly and disabled individuals in Perth. Their services are tailored to each individual client through consumer directed care (CDC).


As the Client Support Coordinator you will provide high level customer service to clients that enquire about services available and notifying existing clients of any service changes. You will be responsible for answering a high volume of calls, high level data entry, scheduling services and general administration tasks.


Responsibilities include:



    • Providing exceptional customer service to clients calling to enquire about services

    • Engagement of new referrals and coordinating services for elderly people

    • Establishing appropriate individual plans for new referrals

    • Understanding aged care funding and packages and knowledge of services available

    • Informing clients of any changes in their current services

    • Developing strong relationships with the clients, support staff and your team

    • Assisting support workers and clinical staff with any queries or information they require about their support shifts

    • Scheduling rosters which deliver consistent service to clients whilst adhering to the awards and legislations

    • Responding to emergencies when required

    • Identifying gaps in training and skills for staff

    • High level data entry whilst speaking to clients and support works on the phone, to record the information efficiently

    • Effective communication with all clients, support workers and clinical staff

    • Complete all administration duties including reports in a timely manner

    • Engagement of new referrals and coordinating services for elderly people

    • Establishing appropriate individual plans for new referrals



    To be successful for the position you will need to demonstrate:



    • A minimum of 2 years experience in a similar role

    • Strong attention to detail and responsibility for data integrity

    • Previous experience using scheduling systems and processes

    • Proven experience to prioritise responsibilities, manage your time effectively and meet deadlines

    • Ability to provide consultation about services available to meet the needs of elderly people

    • Exceptional customer service skills and the ability to listen to people's needs

    • The ability to handle challenging conversations with clients

    • An understanding of Aged Care Legislations and funding packages or willing to learn

    • Demonstrated ability to work effectively in a team and unsupervised

    • Ability to work in a fast paced environment

    • Demonstrated ability to problem solve and use your initiative



    Essential requirements:



    • Full working rights for Australia

    • Willing to apply for a National Police Clearance

    • Availability to work flexible work hours when required
