Hudson is recruiting on behalf of an independent reputable not for profit organisation that provides high quality services for elderly and disabled individuals in Perth. Their services are tailored to each individual client through consumer directed care (CDC).

As the Mobile Care Options Advisor, you will work in a successful and growing team to be the fist point of contact for potential clients to make an initial assessment of their care needs. You will assist the clients with relevant information about support services available and assist them to choose the right care options to match their needs.

Responsibilities include:

  • Providing comprehensive care assessments for elderly people and their families that have been referred to the service
  • Providing home based initial assessments
  • Effective communication with all clients, families, clinical staff and customer contact centre staff in head office
  • Liaising with health care providers ranging form hospitals to community organisations
  • Build strong relationships with clients and understand their needs, funding and lifestyle needs
  • Determine financial viability and liaise with government services
  • Complete all assessment documentation, data and assessments in a prompt manner
  • Engagement of new referrals and convert referrals into services
  • Establishing appropriate individual plans for new referrals with tailored support and lifestyle plans
  • Set Objectives and goals for clients
  • Participate in service reviews with clients and families

To be successful for the position you will need to demonstrate:

  • Current experience within the Community Services sector
  • Demonstrated ability to complete health care assessments
  • A sound knowledge of Aged Care legislations and practices
  • Demonstrated knowledge of service costs and packages for both, residential and home care services
  • Flexibility with work hours
  • Ability to provide consultation on services for the needs of elderly people
  • Proven ability to complete initial health care assessments and financial implications of care services
  • Previous experience with disabled or elderly clients to understand their needs
  • Ability to influence clients to see the benefits in using these services and upsell other services when necessary
  • Demonstrated ability to work unsupervised as your are mostly working in the community and visiting people's homes

Essential Criteria:

  • Full working rights for Australia
  • Willing to apply for a National Police Clearance (must be in date of 3 months)
  • Current WA Driving License
  • Availability to work flexible work hours when required
